What forms of payment do you accept?

Orthopedic Center of Florida "OCF" accepts cash, checks and major credit cards (Visa, MC, Discover).

Will I need to pay my co-insurance and/or deductible at the time of my visit, or will you bill me?

OCF collects all co-payments, deductibles and co-insurances at the time of service or if you are a surgical patient, your patient financial responsibility will be collected at the pre-op appointment. If you are not scheduled for a pre-op visit, arrangements will be made prior to your surgery for any payment due.

Do you take my insurance?

What does it mean when a physician is "fellowship trained"?

This means the physician has participated and completed an additional year of training in a specific field of interest or expertise. Fellowships are not required by the American Board of Orthopedic Surgery.

What do I need to bring with me for my initial visit?

· Photo Identification
· If address is different then what is on driver's license, you will need to bring something else like a utility bill to verify current address.
· Insurance Card
· Co-Payment, Deductible, Co-Insurance
· List of Medications
· Translator if necessary
· Medical History (see forms under Patient Forms)

What do I need to bring with me for my follow-up visit?

· Photo Identification
· Co-Payment, Deductible, Co-Insurance
· List of Medications

· Update Insurance information if applicable
